Instructions to Authors
01.
Original scientific papers in all fields of medicine are welcome. Articles must not have been published or submitted for publication elsewhere.
02.
The manuscript must be written in English and should have a Chinese summary(approximately 800 words, including title, names of authors, abstract, and keywords). The manuscripts should be typed double-spaced on white bond paper. The first letter of the words which appear in a table or a figure as well as the titles of tables and figures should be in capitals (Except prepositions, conjunctions and definite articles).

The manuscript form must follow the "Uniform Requirements for Manuscripts Submitted to Biomedical Journals" by the International Committee of Medical Journal Editors in Vancouver, Canada, which appeared in Ann Intern Med 1997;126:36-47. This format requires-
(1).
Original Article to include abstract, introduction, methods, results, discussion, (acknowledgment), references, tables, legends, figures and Chinese summary in sequence.
(2).
Case Report should contain abstract, introduction, case report, discussion, (acknowledgment), references, tables, legends, figures and Chinese summary in sequence.
(3).
There is no fixed format for text of Review Article (by invitation) but abstract, key words, references and Chinese summary must be included.

The following manuscript components should begin on a new page. (Number all pages beginning with Title page = 1, Abstract page = 2, etc.)
(1).
Title page (the first page) should contain¡G
I. title ¡Ð¡Ð abbreviations not allowed;
II. running title (not exceeding 40 letters and spaces) - abbreviations allowed;
III. name(s) and title(s) of author(s) ¡Ð¡Ð e.g., Min-Chuan Ho, MD, PhD. There should be no more than 5 authors for Case Report;
IV. affiliation(s) and
V. address for reprints ¡Ð¡Ð the name for correspondence and his/her address, phone number, fax number and e-mail address should be also on this page.
(2).
Abstract page (the second page) should carry¡G
I. bstract ¡Ð¡Ð 150 to 250 words, which should included 4 paragraphs: background, methods, results, and conclusion (except Case Report) and condensed abstract (between 50 to 100 words)
II. key words ¡Ð¡Ð no more than 6 key words

The third page should start with the text (or also the acknowledgment).

References should be placed consecutively in order as they are cited in the text. (Identify references in text, and tables by Arabic numerals in parentheses ( ) at the upper right hand site after a comma or a period). Use the reference form adopted by the Index Medicus. The suggested number of references in Original Article should be at least 20, but not exceed 20 for Case Report. Examples of correct forms of references are given below. Note that they should be followed in all details, including punctuation.
(1).
Journals: They should be presented in the folowing sequence: names of all authors, title of article, name of journal, year, volume number, beginning page and ending page.
Example¡GChiang CW, Cherng WJ, Chang FJ, Lin FC, Fang BR, Kuo CT, Lee YS. Effect of heart rate on pressure half-time in patients with mitral stenosis. Chang Gung Med J 1992;15:72-7.
(2).
Books: They should be presented in the sequence of name(s) of author(s), title of article, name(s) of editor(s), title of book, edition (volume), place, publisher, year, beginning page and ending page.
Example¡GSchiebler GL, van Mierop LHS, Krovetz LJ. Disease of the tricuspid valve. In: Moss AJ, Adams F, eds. Heart Disease in Infants, Children and Adolescents. 2nd ed. Baltimore: Williams & Wilkins Co., 1988:134-9.

Tables: Each table should be written on a separate sheet with a heading and number (Roman numerals).
Vertical lines should be omitted.
Footnotes: Limit use of footnotes. Use symbols in the following order: *,,, II,£k,**,,¡K¡K¡C

Illustrations and photos should be 3¡Ñ5 or 5¡Ñ7 inches. Photos should be on smooth photographic paper and numbered on the back with soft pencil with the title and the orientation indicated. Line drawings should be drawn with black ink on white paper. Photomicrographs should give the magnification. Type the legends for illustrations on a separate sheet of paper. The number of figures and tables should not exceed 4 in Case Report.

All measurements should be presented in metric units, m, cm, mm, mmHg, mm, nm, l, dl, ml, ml, kg, g, mg, mg, kcal, ¢J, etc. For other symbols and abbreviation, please refer to IUPAC-IUB Document No. 1 (Arch Biophys 1966;155:1-12.)

Terminology: All abbreviations and acronyms are to be spelled out in full on their first appearance in the text, e.g., nasopharyngeal cancer (NPC). Herbs are to be presented in the text by their common name, pharmaceutical name, botanical name, or oriental name (capitalized where they are part of a formula name): herb glossaries are to include all of these entries. Oriental medical terms are to be defined through their use in the text, or with an accompanying glossary; terms used in acupuncture are to conform to Standard Acupuncture Nomenclature as authorized by the World Health Organization.

Manuscripts, including tables and figures, should be sent in triplicate (original and two copies, photos must be original), and also on a PC-Word disk file, as well as a consent form signed by all authors.
11.
If the manuscripts do not meet the requirements of the Journal, the editors are authorized to make changes or return the manuscripts. The article once published is copyrighted by the Journal. No part of the article may be reproduced by any means without written permission.
12.
The manuscript is accepted for publication according to the sequence of approval. If the author(s) hope to publish immediately, they should assume financial responsibility for the printing cost
13.
The author(s) will receive 50 copies of reprints of the article without charge. If more reprints are needed, they may be ordered during the first proofreading.
14.
Manuscripts should be sent to the Editorial Board, the Chang Gung Medical Journal, Chang Gung Memorial Hospital. 199 Tung-Hwa North Road, Taipei 10507, Taiwan, Republic of China.
 

Transfer of Copyright
It is assumed by the editor that articles emanating from a particular institution are submitted with the approval of requisite authority. A signature below certifies compliance with the following statements. All authors must transfer ownership of accepted articles to the Chang Gung Memorial Hospital (CGMH) and must sign the copyright form below.

I affirm and represent that the submitted article is my original work; that none of the information has been published previously or is under consideration by another publication; that upon publication nothing contained in the article will constitute an infringement of any copyright; and that its contents are neither libelous nor constitute an invasion of privacy.

I agree that the CGMH may publish the article in a forthcoming edition of the Chang Gung Medical Journal and hereby assign and transfer all rights, title, and interests thereto, including the right to claim copyright throughout the world and for the full term and all extensions and renewals thereof, to the CGMH, agreeing to hold CGMH harmless from any damages resulting from the above representations.

This transfer of copyright allows the CGMH to provide for the widest possible dissemination of the published article through activities such as the distribution of reprints and authorization of indexing and abstracting services in print and electronic database formats. Without this transfer of copyright, such activities by the CGMH and the corresponding spread of information are limited.

I acknowledge that the publication of the submitted article and other considerations agreed upon constitute the entire consideration due me for all rights to copyright.
